I'm, an amateur, so I don't and can't put in the hours a pro does; but I do find taking short (5 - 15 minute breaks) breaks every hour really helps.
There's is also something else that happens occasionally. I will be playing some piece of music that I have been having trouble with, when I realize that for the last couple of minutes my mind has completely wandered. The really cool part is that I also realize I was still playing the music and playing it well. This doesn't happen often, but it is great when it does.
