I have had the experience of a student playing well under pitch due to lack of air support, but usually not *that* flat. Without seeing/hearing it's kinda hard to say, but no air and weak embouchure jump to mind. Young players often feel like they have to stuff their entire face into the mouthpiece. You might try getting your student to match pitch on the mouthpiece first, then on the horn. Also, asking them to sing the pitch before they play it is a good indicator of being able to match pitch at all. If they can't sing it, they're gonna have a tough time playing it.
