A fellow my wife knows obtained permission from the author of a choral piece to make an instrumental arrangement (a very nice one it was, too) of one of the author's works. Sometime later, the author passed away and the copyright became part of his estate.

When the arranger submitted his work to the family for approval, they refused him permission to publish because it wasn't in a choral form. Apparently, they wanted the now-deceased author's name to be associated only with choral music.

All of this in spite of the fact that the arranger made his intentions very clear to the author when the original permission was obtained.
