Seriously, I suspect that the greatest risk of getting metal poisoning will happen if you have any abrasions where you contact the metal. Nicks or dings in the mouthpiece will likely exagerate the problem.

I have had more than a few students who had metal allergies and the key to keeping their chops healthy was a good, smooth, well plated mouthpiece. You may be ok with an unplated mouthpiece, but you may start having cold sores. Personally, I'd stick with a silver or gold plated mouthpiece and be safe.
