This problem has plagued me for the last few years, and now that I'm studying with a teacher again we've nailed it down to just what lisa said. My embouchure is not firm and consistent in the corners, with the result that I rob my low range of power (and clarity--sound familiar?) and my high range of endurance. I'm using pressure to overcome the fault, and I've gotten reasonably good at it, except that getting a steady tone in the middle register ain't so easy. It's too low for pressure to work and too high to tolerate those weak corners.

I'm doing specific embouchure exercises right now to help the problem. EVERYTHING will be better when I work through it.

But I don't want to tell you what exercises I'm doing, because they may not be right for you. I would recommend some close scrutiny from a good teacher for a few lessons, however.
