I have to admit you've asked some questions I never considered when mine were taken out over 25 years ago.
An oral surgeon is the best way to go, it was back then. A little numbness on the lip normally goes away just like when you go to the dentist to get a filling. You'll know when to start playing. That's when it doesn't hurt anymore. Nothing scientific about that part.
I had mine taken out 1 side at a time back when I was in graduate school. One side was at Christmas time and the other during the summer. Both healed up fine and I began playing right away, just had to wait until there wasn't much hurt.
