One approach would be to find more"game time" situations. Nailing excerpts in the practice room is just that. Finding real situations in which to "test your mettle" can be tricky, but look for those opportunities.
I read somewhere that the body has the same reaction to imagined pressure situations as it does to real pressure situations. Imagining the concert setting while practicing(tux, audience, hot lights, different time of day) can produce similar physical reactions in the body(elevated heart rate, dry mouth etc). While I'm not really into the whole "inner game" thing, I think there is some merit to this kind of approach.
On a different tangent, is your equipment letting you down at all? Fighting with the horn is not desirable at any time, much less in high, exposed, delicate stuff. Take any measures you can to correct equipment deficiencies.
I don't know the Adams, but I hope some of this helps.
