Embouchure.
If the upper lip becomes to tight it will "bisect", causing the double buzz. Generally happens in the upper register after extended practice? Are you "smiling" into the mouthpiece? Are the corners of your mouth upturned? they should not be, this tightens the upper lip.
Try working on embouchure building exercises like long tones and lip slurs, concentrating on keeping the corners of the mouth firm- straight or downturned (no smiling!) , using a minimum of pressure. (mouthpiece to lip pressure, not air pressure!) To much pressure can exacerbate the problem.
