People have pretty much hit the nail on the head so far. What's happening is your embouchure doesn't really know what to do (your jaw and lips don't know where they should be) to sing whatever pitch you're trying to produce. So, the lips just tighten up, and everything else moves around to compensate to get the pitch out. In your case one of your lips seems to dominate, producing a double buzz (most of us just get really horrible tone).
Listen to what Mr. Frederiksen said, but start out with just the mouthpiece. Make sure you know what the desired pitch is (play it on a piano, sing it, whatever) and copy the sound on the mouthpiece. Try to make a fat, thick sound (as little hiss as possible). Remember, if you can't do it on the mouthpiece, you haven't a prayer on the horn.
