There are some production horns that use the system extention slides that you describe.
1. Getzen G-50 aka CB 50 is a CC tuba that comes with a full set of slides to convert the horn to BBb. There are also 5th valve adjustments that can be made to make the horn play differently.
2. Meinl Weston 2145, 2155, 2165 are CC tubas with optional slide extenders to convert to BBb. I believe Matt Walters is the person who originally posted about this. I believe that MW slide sets are expensive and hard to come by, but the do exist.
Those are the only tubas that I know of that are designed for switching between two different keys (CC and BBb).
