A pre-made mouthpiece will, generally, be cheaper than a custom-made mouthpiece and, under ideal circumstances (assuming the manufacturer can repeat the mouthpiece shapes), a pre-made mouthpiece will be easier to replace if lost or damaged.
With some experimentation, it is possible that a custom-made mouthpiece could be made to provide close to an ideal match with the player and a specific horn. It is also possible to spend a great deal of money trying different custom shapes without finding anything that works better than a pre-made mouthpiece for a given player and horn.
