Everything "been there" has said and one really off-the-wall one, but sometimes it's the problem:

When you play high notes, does your facial expression change dramatically? That is, do you raise your eyebrows the higher you go? If so, that might be the limiting factor (i.e. you can't raise your eyebrows any higher). Endeavor not to alter your facial expression and it can be amazing how this will free up the high range. If you can see your reflection in your tuba as you play, pay attention. Or use a mirror.

We sometimes make all sorts of bizarre body movements that have absolutely nothing to do with the sound when we're playing, but become part of it nonetheless. Recognizing what's happening and disassociating it from playing can be wonderfully liberating.
