There are two parts to sight reading, and one has been well addressed by the previous comments; that one is to find literature that exposes you to everything imaginable you could run into with rhythm and note patterns.

The other is equally important: you must be able to do this and _stay_with_ the ensemble, which is an entirely different skill. If you follow your "internal beat" and play the rhythms and notes perfectly, but the ensemble doesn't happen to be following your personal internal beat.....you won't be with them. You have to have some of your attention on getting what's on the page correct, and the other part of your attention on listening to what's going on around you and fitting into it. For many people, that's the harder part. Don't neglect it....find a group to go to that will let you sit in and sight read with them. In that case, it's much more important to stay with the group than to get all the notes. Groups don't mind if someone doesn't get all the notes; they do mind if someone is playing all by himself off in left field somewhere.
