I am really old-fashioned, and have positioned the fifth rotary valve on all of the tubas that I have "built" (so to speak) for myself AFTER the main tuning slide.

It should be operated with the right hand thumb, unless your instrument plays so well in tune that NO manipulation of slides is required (Very luckily, I own ONE like this.) or if you plan to manipulate slide(s) with a lever from your LEFT hand.
