First, a question:
DO you practice buzzing (only with the mouthpiece).
Sometimes, when the note isn't well center on the mouthpiece, you can play the good note as well, but without centered sound on the mouthpiece, you don't play right on the good pitch of the harmonic of your instrument. A good way to avoid crack the sound, is to well know your instrument and the natural pitch of your instrument (you should adjust pitch by a good setting of the slides). Try to play directly on the centre of your instrument (that notes arrives naturaly). Take a deep but very relax breath and let it going out in the same way you took it (but the air should go out instantly).

About the mouthpiece question, wich mouthpiece are you using? I recently changed my mouthpiece and the result is great...

Hope I helped you,
Have a nice day.
Martin
