I have a 191 5-valve in gold brass. The fifth valve doesn't cause any resistance problems that I can detect. In fact, some on this board have complained that the 191 does not have enough resistance. I think it is just about right.
The fifth valve is really only useful for Eb down. But it makes playing those low notes a whole lot easier. I think it is worth the extra cost.
