Many thanks to each of you who posted a comment on the important topic of performance anxiety and panic attacks. I'd like to summarize some recommendations, which I think loosely fit into 4 categories:

----------------------------
1) Using Reason, introspection, and Faith

Play for God
Don't think about the audience, but have a good time and play for your own enjoyment
Books by Dr Robert Benson http://www.stress.org/benson.htm
Articles by Doug Yeo with some faith-based recommendations http://www.yeodoug.com/yeoarticles.html

---------------------------
2) Using physical techniques

Yoga or other relaxation techniques
Chi Gong or Tai Chi
massages
Alexander Technique

--------------------
3) Foods and Herbs

Eat bananas prior to performance
Valerian Root (tea or herbal supplement)
Gotu Kola (tea or herbal supplement)

-------------------
4) Prescription drugs

Adrenaline blockers (beta-blockers) small doses - usually 10mg, up to 40mg
Inderal (short term acting beta blocker)
Propanolol (long acting beta blocker)
Tenormin (generic for Propanolol - cheaper)

Drugs specific for anxiety and panic attacks
Paxil (10mg) http://www.paxil.com
