This subject crops up quite a lot in the archives, but just to give a short answer a fifth valve is intended to correct the intonation problems associated with pressing down several valves at once by allowing alternative combinations. They seem to be comparitively rare on BBb tubas but are farely popular on CC where they are usually tuned to a flat whole step. The extra tubing also allows you to play the note a half step above your first pedal without using any tricks (eg. false partials). Only really worth paying to have one added if you are unhappy with your low range intonation, or if you have to play a lot of low B naturals :) Can't say how much it costs as I've never had it done.
