Here is a list I copied from Amazon. It's pretty exhaustive. As far as rates of pay go, I know the New York Phil. is one of the highest paid orchestras in world, if not the highest. As for the rest, I can only assume that the top flight ensembles like Chicago, Philly, Boston and LA are up around the top as well. I think it's pretty impossible (and stupid) to try and create a rank order. Everyone's got their own personal favourite/s.
