My wife and I both suffer from this. My situation so far has been limited to the popping sensation felt occasionally after long periods of performance.

My wife's situation is at the point that it was cartially causing her headaches and tensionbecause of her attempt to fight through the situation, and not being aware of the actual cause of her problem.

A dental surgeon gave her a mouthpiece to wear at night to help keep the jaw in-line and to help her keep from gnawing on her own teeth in the hours she sleeps. (I know the gnawing is not really connected to the TMJ, but the thought was to help kill two birds with one stone).

She has shown some signs of improvement, but I wonder if she is helping symptoms without attacking the cause at this point.
