This used to happen to me in marching band when I played very loudly with a lot of air pressure. Mr. Zerkel's suggestion about keeping your tongue low should help. Also, I would make sure to relax as much as possible while breathing and while blowing. Mr. Frederiksen is correct when he states that we need a large amount of air under low pressure. You might think of slowing the air down, or just letting it come out without any assist from your abdominal muscles. The natural air flow generated when you take in a large relaxed breath should be enough in most situations.
