1) Northwest Airlines/KLM/Mesaba
2) No, though they ARE very strict about the number of bags you bring.
3) Yes and No. Of the 35-or-so times I flew in 1999-2000(about half of those with a horn), I was able to get the horn loaded at the gate all but one time(the minimum-wage-ticket-agent told me that it was "too icy out for the horn to be loaded at the gate").
4) The one time I was unable to get the horn loaded at the gate, it was completely totalled.
5) Despite the fact that they totalled my horn, I'd still give them a 7-and-a-half. Within 2 weeks I got a cheque to cover all repairs, and purchase a new case.
