Lots of good answers to date. I agree that just playing up there a lot will help. The two things I think help me the most are:

1) trying to avoid using too much air pressure until it is absolutely necessary. The pressure will increase as you go higher, but don't help it along.

2) concentrate on playing some really beautiful music up there. Brahms and Mahler songs, lots of Horn music e.g. Schumann-Adagio and Allegro, Rochuts at pitch...this will help keep your mind focused on the music and allow your body to do what it needs to do to produce the results you seek.
