TMJ + High Register - Problems

I have been dealing with TMJ for the past few months, and my jaw muscles are very tight...and sometimes get very swollen and sore. I used to have the capacity to play in the upper register of the euphonium for an extended period of time, but now I'm lucky if I HIT a B flat. Using a mirror during my practice, I've found that to compensate for the swolleness and discomfort of the upper register, I've been subconsciously pulling my corners outward and upward - the "smiley" embouchure. This is a difficult problem to fix because I've been doing it for a while. Does anyone have any suggestions if you have dealt with this problem before? I must regain my sonorous range once again!
