I believe carpal tunnel syndrome can result from using your fingers a lot with the wrist bent, as is commonly done by Fender Bass players. Some tubas, both piston and rotary, force the player into a similar awkward hand position, and others do not. Of course it also depends on the length of your arms and the size of your hands. What's right for one person is often wrong for the next. Just try to avoid trouble by keeping your wrist straight while playing, and use whatever instrument makes you comfortable.
