I recommend you use the Breathing Gym and Mouthpiece Warm-up by Sam Pilafian and Pat Sheridan.Although i can't diagnose your problem, I have found the breathing exercises very curative . Also the buzzing using a rim, not a mouthpiece will help.I use a cut out Schilke but you should buy a rim from Brian Fredrickson.
Also check the placement of your mouthpiece. I moved mine too high on my face over time and man it has been tough re-establishing a good focus with efficiency.
Also use simple melodies, eg. Amazing Grace . I believe this falls under "wind and song"
