As an overview, here are some of the general things I have used to develop my upper register:

1)Concentrating on breath and the air flow
2)Adjusting my embouchre (more than typical adjustments to the embouchre when moving between ranges)
3)Trying different mouthpieces
4) Modifying my warm-ups to prepare myself for playing in the upper register

Incidentally, I have generally found the most success when combining #1 and #4, but this isn't consistently true for me.

Something else I should mention is that my difficulty isn't with hitting the notes, but that the sound is strained. I want to fill out the sound a lot more.
